Lender Request for Cancel/Reissue Form
The Utah Higher Education Assistance Authority (UHEAA) has revised the Lender Request for Cancel/Reissue form. A lender may request a cancel/reissue only on loans which have not been disbursed and only when the borrower requests a different lender or the incorrect school code is entered on the application. Requests for cancel/reissue for any other reason will no longer be accepted because such changes cause significant problems for schools.
After receiving the properly completed Cancel/Reissue form from the lender, UHEAA will cancel the loan, reissue a new guarantee, and send a Notice of Guarantee and Disclosure Statement to the lender. UHEAA will notify the school of the change by sending a Unique Loan Identifier Change form.
Copies of both forms can be obtained by contacting Teri Vig at (801) 321-7223. Lenders may make additional copies of the Cancel/Reissue form as needed.
