Revised Total and Permanent Disability Cancellation Request Form

The Department of Education (ED) recently released a Dear Guaranty Agency Director (DGAD) letter revising the Total and Permanent Disability Cancellation Request form. ED revised this cancellation form in an effort to deter incorrect or fraudulent certification of the form. The changes are also designed to facilitate random checking of disability certifications.

The new Total and Permanent Disability Cancellation Request form must be sent to all borrowers requesting total and permanent disability cancellations on or after January 1, 2000. The form recently approved in an April 1999 DGAD letter may still be processed after January 1, 2000.  However, the revised form must be used when providing disability cancellation applications on or after January 1, 2000.
