Request for Disbursement Cancellation or Revision Form

The Utah Higher Education Assistance Authority (UHEAA) has revised the Request for Disbursement Cancellation or Revision form in an effort to improve the form and simplify the cancellation/revision process. Changes to the form include the elimination of unnecessary information and the addition of a lender section.  Schools and Lenders may use this new form to cancel or revise a single disbursement or a full loan.
