Approval of Deferment and Cancellation Forms

    The U.S. Department of Education (ED) recently issued a Dear Guaranty Agency Director letter, GEN-01-07, announcing the approval of eight deferment and two cancellation forms pertaining to the Federal Family Education Loan Program (FFELP).  On November 1, 2000, final regulations were issued making several changes to the existing regulations governing deferment and discharges (see UHEAA Bulletin Volume 2000 - Issue 90).  These regulatory changes become effective July 1, 2001.  Since these forms have been revised to meet the July 1, 2001 effective date of the regulatory changes, these revised forms should be distributed and used in response to borrower's requests as of that date.  Other forms issued and approved prior to that date may continue to be processed.  The forms are as follows:

    Deferment forms-
     
    1. Education Related Deferment Request
    2. In-School Deferment Request
    3. Parental Leave/Working Mother Deferment Request
    4. PLUS Borrower With Dependent Student Deferment Request
    5. Public Service Deferment Request
    6. Temporary Total Disability Deferment Request
    7. Unemployment Deferment Request
    8. Economic Hardship Deferment
 Cancellation forms-
  1. Loan Discharge Application: False Certification of Ability to Benefit
  2. Total and Permanent Disability Cancellation Request
    The revised forms are available in PDF format as an attachment to this letter.  The forms are also available on both NCHELP's and UHEAA's web sites at http://www.nchelp.org and http://www.uheaa.org/BULLS/grinfo.htm.  No changes may be made to the forms except those expressly authorized by ED.
