Approved Federal Teacher Loan Forgiveness Forms

    The U.S. Department of Education (ED) has released Dear Guaranty Agency Director Letter, GEN-01-15, dated November 2001, announcing approval of the Teacher Loan Forgiveness Application Form and the Teacher Loan Forgiveness Forbearance Form.  These forms are to be used in determining eligibility for loan discharge or forbearance in accordance with the Teacher Loan Forgiveness Program regulations in 34 CFR 682.125 and 34 CFR 682.211.  These forms are now available from Utah Higher Education Assistance Authority (UHEAA) at http://www.uheaa.org.

    In addition to these ED approved forms, UHEAA encourages lenders to use the "Teacher Loan Forgiveness Discharge Request" form (copy attached), developed by the NCHELP Teacher Loan Forgiveness work group, when processing such discharge requests from borrowers.

    A borrower may qualify for forbearance during qualifying teaching service in annual increments if the lender determines, at the time of the borrower's annual request, that the expected cancellation amount will satisfy the anticipated remaining outstanding balance on the loan at the time of the expected cancellation.  A borrower also may qualify for forbearance while the borrower's application for loan discharge is in process.  In processing a borrower's request for forbearance while qualifying for a Teacher Loan Forgiveness discharge, lenders and servicers should follow the ED guidance contained in 34 CFR 682.215(e) and 682.211(h)(2)(ii)(C).

    Some teachers may qualify for forgiveness of up to $5,000 of student loan debt if they meet ED's eligibility requirements. See UHEAA Bulletin Volume 2001- Issue 06 for additional information on borrower eligibility criteria for the Teacher Loan Forgiveness Program.
