Loan Discharge Application for Total and Permanent Disability Form
In a UHEAA Bulletin dated June 3, 2002 (Volume 2002 - Issue 27) specific guidance on the new procedures for discharging student loans because of total and permanent disability was provided. The U.S. Department of Education (ED) recently released Dear Colleague Letter (DCL), GEN-02-04, approving the new Total and Permanent Disability Discharge Application form to be used for Title IV loans. This new form is the only one that may be distributed for use for total and permanent disability discharge requests on or after July 1, 2002.
The new discharge application is attached to this Bulletin in PDF format for schools and lenders to use. Please note that loan holders should pre-print their name and address and provide a contact number for borrowers to call in the boxes provided for that purpose in Section 8 on page 2 of the form.
