PLUS Loan Credit Approval Request Form

    The Utah Higher Education Assistance Authority (UHEAA) recognizes that parent borrowers and schools occasionally seek a preliminary credit approval prior to the completion and submission of a PLUS loan application and school certification.  To better facilitate this process we have attached an updated form which can be used to request a preliminary credit approval for PLUS loan borrowers.  This form is also found on UHEAA’s website, uheaa.org, under the link http://www.uheaa.org/uslphome.htm for Student Loans and Borrower Benefits.

    Schools may have this form completed and signed by the parent borrower then faxed to UHEAA.  A determination will be made on a parent’s eligibility for credit to qualify for a PLUS loan prior to the submission of the Federal PLUS Loan Application and Promissory Note, or the initial Federal PLUS Loan Application and Master Promissory Note.

    Please note that requesting a determination of credit worthiness prior to the submission of an application (which authorizes and generates a credit check) and certification is not required but is merely an alternative approach a parent may wish to use before applying for a PLUS loan.
